| Miami Beach Community Development Corp. HOPWA
Project-Based Housing Program has a
limited number of openings for Studios and One Bedroom apartments in its
Project-Based Housing
Program and is seeking to fill
these openings through
a Project-Based
Waiting List. Applicants will be
placed on the Waitlist by random lottery. If you have been diagnosed
with AIDS and are low income, you may be eligible for placement
on the Project- Based Waitlist.
HOW DO I APPLY?
The
Application Package to be submitted is made up of two forms. You must
(1) fill out and sign a
HOPWA Project-Based
2007 Waitlist Application Form and (2) have your
physician fill out and sign a
Medical Verification of AIDS Diagnosis form. You must then submit
both of these forms together with a postmark no later than August 31,
2007 to MBCDC HOPWA Program located at 945 Pennsylvania Ave 2nd
Floor Miami Beach, Fl. 33139 Attention Mrs. Zaily Pineda.
Admissions Criteria
1. At least one member of the household must be:
a. AIDS diagnosis
certified by a physician and
b. Disabled, according to
Social Security or their primary physician
2. Total annual household income can not be greater than the
following:
